New Patient Registration

We are very happy to take on new patients from within our practice area. There are some simple forms to fill in which can be done at the reception desk of any of our surgeries. If you need any help to complete any of the paperwork, do please ask for assistance.

Catchment Area

If you know your NHS number or have an NHS card it will speed up the process. We will also need to know the name and address of your previous doctor. We run personal lists on the Roseland which means you will be allocated your own doctor. However, you are free to choose which doctor you see and you may be seen at any site. We would ask that you try and see the same doctor for the continuous care of a particular problem, where possible.

Due to the unpredictable nature of illness there may be times when you are asked to see one of the other doctors in the practice for expediency sake. However, as a general rule we try and maintain continuity with one doctor so you can build up confidence and trust in your GP.

If you are registering and have an appointment to see the doctor at the same time it would be helpful if you could turn up 10 minutes before your appointment to complete the paperwork. Alternatively you can complete and submit the registration form online.

If you are a visitor and need to see a doctor you can sign on as a temporary resident via our temporary registration forms.

You do not need to provide proof of your address or photographic ID in order to register with this practice, but if you do have it, it can be helpful to ensure that we have the correct spelling of names/addresses etc.

All new permanent patients are offered a New Patient Check with the practice nurse when they first join the practice.
